Important Reminders

 

**All new students are required to submit an immunization record and physical exam.

-Forms are available on the left hand menu under "Health Office Forms"

 

**Pre-K students must receive a flu vaccine for this coming flu season.  

 

**Incoming 6th grade students are required to submit updated immunization records that include Tdap and meningococcal vaccines. Eleven year old students without this immunization will NOT be allowed to attend school as per NJ Department of Health and Senior Services regulations.
